Surgical management of gallbladder polyp

Abstract

Within the gallbladder diseases in our environment we have cholelithiasis as the most common pathology, but the diagnosis of gallbladder polyps by ultrasound or histopathological study is more frequent every day. The importance of these polyps lies in the possibility that it is a premalignant or malignant lesion and that proper management is not provided. Hence the need for a comprehensive literature review, in order to know when to decide surgical treatment. A systematic, orderly and thorough review of 74 scientific articles was carried out, but fifteen articles that met the inclusion criteria were cited. With what we found, we agreed that the surgical indication “laparoscopic cholecystectomy” of gallbladder polyps, when the ultrasound reveals a solitary, sessile, vascularized and greater than 10 mm polyp, in patients over 50 years of age. Conventional cholecystectomy is recommended with the above criteria that invaded the mucosa with a wall width greater than 4 mm. Conservative and expectant management, ultrasound control, at 6 months, and then a year for 3 years in 5 - 10 mm lesions are recommended
